The Serie D is the highest level of the Italy football association Lega Nazionale Dilettanti. The association represents over 12,000 football players and 400 football teams throughout Italy. The Serie D is just below the Serie C football (the third and last professional league since 2014-15) and is therefore considered the fourth largest league in the country. It is organized by the Roman Comitato Interregional (Interregional Committee), a "League in the League" within the LND.

In 1948, Division 3's three leagues had to be reorganized due to an ever-increasing number of regional teams. FIGC decided not to ban the surplus teams to regional championships. The winners and runners-up of the 36 C Series championships were selected and added to the new third division in four groups. The rest of the teams joined the new Promozione, which in 1952 changed its name to IV Series (Fourth Division) and 1959 Series D.

From 1959, each player in Serie D had to opt for a semi-professional status by signing a specially issued status assignment form. The championship was thus included in the Lega Nazionale Semiprofessionisti, which is known today as Lega Pro. The D series was reorganized in 1981 when the championships were reduced. The league name changed to interregional. Players have lost Semi-Pro status and converted to amateurs. The championship then went into the Lega Nazionale Dilettanti. From 1992 to 1999, the name changed in Campionato Nazionale Dilettanti, before he finally returned to the current name of the D Series. With the merger of the two divisions of the Lega Pro at the end of the 2013/14 season (as decided by FIGC and Lega Pro in November 2012), the Serie C, the Serie D and the underlying leagues rose by one level in the pyramid system, reduces the number of leagues in Italian football to nine.

The first-placed team in each division is promoted to Serie C each year, replacing 9 Lega Pro teams descending into Serie D.

If a newly promoted D-Series team does not meet the requirements, Lega Pro will ask the second-placed team in that team's division to fill the vacancy. If this fails, the third-placed team can fill the position and so on.

In recent years, one or more teams from the professional leagues have usually not met the legal or financial requirements to participate. This usually leads to vacancies, which will be announced in the summer break when the new season is due. When teams move up to fill the gap created by these failed teams, or teams in the lowest professional division fail, spaces are created in Series C (or in the Lega Pro Seconda Divisione before the 2014-15 season), which have to be filled.

For example, in the 2007/08 season, there were nine such failures, and nine Lega Pro Seconda Divisione spaces were created. Four of these seats were filled by recalling teams that had played in the Seconda Divisione but relegated to Serie D for the next season. The other five vacancies were filled by Serie D teams that participated in the D Series playoffs.
